Four steps:
1. Tune Guitar (click here if you're not in tune)
2. Learn A7 chord (Just 2 fingers, easy as it gets.)
3. Learn D chord (same 2 fingers moved over 1 string plus add another finger.
4. Strum at each "/" mark over a word, and at each chord symbol, and Sing (that's it, you're playing).

The secret is to be able to keep the song going "in time" when you change chords. At first you'll have to think about where to put your fingers and you'll probably want to look at the fret board when you change chords. As soon as you can stand the thought of it; don't look. You'll know if you don't get it, you'll hear it. Smooth chord changes happen automatically when you know the fingering for the coming chord. You don't have to think about it. You strum confidently on the first beat of the next chord. This can only be achieved with considerable practice. In the meantime you must have faith that it will get better. It will.
The "/" mark means strum here. Play slowly at first so you can keep the rhythm going steadily. The beat is really the most important thing. You can keep the beat without even playing the guitar if you want to. You can keep the beat by tapping your foot or by tapping your fingers on your guitar. Keep the singing going steadily whether you can make the chord changes fast enough or not. One more thing, notice that this song doesn't start on the downbeat. Play the D chord first, then start singing.
I find that it's best to focus on the singing and let your strumming follow your singing. To get the beat started right I sometimes sing in my head before strumming. This isn't a music theory book, it's a how-to-do-it book. With some things it's best to just do it without thinking about it so much. Feel the song. Feel the beat. Tune your heart then play it again.
